Google Form

Google Forms is a great way to collect information in a visually pleasing way and an alternative to a Canvas survey. Google Forms allows you to design your form, add your own questions, and choose themes to make it visually pleasing. Because it is a Google product, it is free, but requires a Google Account/Subscription. 

Google Forms information is easily collected into a Google Spreadsheet for evaluation and may also be shown, visually, through a Google Chart.
